
Every three years Valley Health System conducts a Community Health Needs
Assessment (CHNA) to identify health needs, gaps in services, and the
impact of behaviors on health and wellness. A key part of a CHNA is gathering
direct input from individuals living across the region. Between January
1 and February 28, 2019, over 2500 individuals completed a community health survey.
Results from that survey, along with feedback gathered from healthcare
providers, community leaders, non-profit service organizations, and other
community stakeholders, will be used to identify the most significant
health challenges facing our region. Valley Health will then develop strategies
for addressing the identified needs.
The survey results and the implementation plans for addressing the identified
needs will be made publicly available in late 2019. Valley Health would
like to thank everyone who has participated in the CHNA process up to
this point. We look forward to working together to make our communities
healthier!
